Your comfort and convenience are important to us, and we strive to make your patient experience a positive one.

Before Your Appointment

Checking In

Please bring your insurance card(s) and photo ID to your appointment.

New Patients: Please arrive 15 minutes prior to your appointment to fill out new patient forms, unless you already have them completed.

Existing patients: We require forms to be updated on a yearly basis, even if you have had no changes. Please call and notify us of any changes to your insurance, address, phone number or primary care physician.

All patient forms are available to download and complete before your visit through the links on the right hand side of this page.

Requesting Medical Records

We do not charge to provide copies of medical records to our patients or other medical providers.

To request a copy of your medical records, you must complete an Authorization for Release of Medical Information form and return it to our office. If you request that your records to be sent to another provider, they will be sent directly to that provider. Most records requests are completed within 7 working days, if not sooner.

Copays

Payment is due at time of check-in, prior to receiving services.

Medical Forms

We are happy to assist patients who need a physician to complete forms related to their vascular medical status. Patients are responsible for supplying the
necessary forms and completing the patient sections before bringing it to us. Understand that it may take a few days for us to have the forms ready for you to pick back up; we will notify you when they are ready.

Appointment Cancellations

When you schedule an appointment, we set aside enough time for the doctor and/or ultrasound tech to provide you with quality care. If you need to cancel your appointment, please contact our office right away, and no later then 24 hours before your scheduled appointment. This gives us time to schedule other patients who need to see a doctor.

We understand that circumstances can sometimes change at the last minute. If you miss a scheduled appointment due to extenuating circumstances, please contact our office at 503-292-9565.

Insurance and Payment

Will insurance cover your vein treatment?

Because coverage is based on medical necessity, most patients find that their insurance does not cover vein treatment for “cosmetic” issues such as spider veins and superficial reticular veins.

Insurance may cover the treatment of varicose veins if specific symptoms of vein disease are diagnosed. However, you will likely be required to try conservative therapy under a physician’s supervision prior to treatment.

Coverage and authorization times vary, so it’s a good idea to call your insurance company before your first visit. If your treatment is covered, our billing staff will be happy to review the process of how to submit an insurance claim with you.

Easy payment options

The cost of vein treatment shouldn’t be a barrier to your comfort, confidence and well being. Northwest Vascular Consultants offers two choices for paying out-of-pocket for your varicose vein or spider vein treatment.

The first option is an up-front payment. If made at the time of service, you will be eligible for a 20% discount on the total cost of your treatment.

The second option allows you to pay in regular monthly increments. Half of the total balance is due at the time of service, and the remainder can be made in continuing payments of $100 (or more) until the balance is paid in full.
